Main duties and responsibilities:

Monitoring the client’s health status and adjusting care plans as needed based on client needs
Working with the family to ensure that they are comfortable with the care plan and know how to provide care if needed
Scheduling appointments with physicians and other healthcare professionals to ensure proper treatment is being received
Ensuring that the home environment is safe for the client by ordering new equipment or making repairs to items such as stairs or cabinets
Coordinating with other service providers such as physical therapists, occupational therapists, and social workers
Assessing the client’s needs and working with the family to develop a care plan that meets those needs
Managing all aspects of the client’s care, such as hiring and training caregivers, helping with bathing and dressing, preparing meals, and administering medications
Cultivating strong relationships with the client and his or her family in order to understand their needs and concerns
Communicating regularly with case managers and other home care staff members about the client’s progress and needs
Required Experience in Handling:

-Must handle patients with peg tube, tracheostomy, colostomy and mechanical ventilator and CPAP Machine
– Child Care
– Newborn
-Advance Elderly care
